Form 1095-C Requests

As a result of the Paperwork Burden Reduction Act, Reynolds furnishes Form 1095-C to associates and former associates only upon request.  As a reminder, it is not required to complete your taxes. Reynolds will still file with the IRS at the employer level, as that requirement has not changed, and it will continue to follow the appropriate Form 1095-C guidelines in states where the form is still required to be sent.

To request Form 1095-C, email benefits@reyrey.com.  Please provide your name, date of birth, mailing address, and year(s) that you would like the form for.  You may also indicate that you wish to have the form emailed to you rather than mailed.  If you do not request a form, it will not be provided to you.
